Official Forms, Language and Abbreviations

 

Completing official forms, application forms and other official documentation requires an understanding of the language involved. Properly understanding the language and what is being asked of you can save a lot of time and inconvenience. As forms are used for a large variety of purposes the language used, is to some extent, unique, however their are a number of common terms which are used.

 

Some examples are;

 

Forenames/first name/given name/Christian name (s) - (e.g. Leigh, Leigh Ann)

Middle name - (e.g. Ann)

Family name/surname - (e.g. Rogers)

 

Age

Sex

Marital status:

Married

Single

Divorced

Seperated  (living legally apart from husband/wife)

Widowed    (husband/wife has died) 

 

 

Date of birth

Date of purchase  (the date that you bought something)

 

Address:

Permanent  (your usual address in your home country)

Temporary  (where you are living at present)

Home (you or your parents home address)

Work (the address of your work)

 

Tel. No. (telephone number)

Passport No.

Bank Acc. No. (Bank account number)

Your ref: (your reference number in business letters)

Our ref: (the reference number of the company you're writing to)

 

You will often see the following two expressions:

 

BLOCK CAPITALS (write in LARGE CAPITAL  letters)

PLEASE PRINT (do not use cursive writing)

Complete the forms and then show them to someone in the class to check over.
